✓ Building Permit Form Duly filled out by the applicant or licensed professionals (Civil Engineer/Architect), signed and sealed including ancillary permits: Sanitary/Plumbing Permit Electrical Permit Mechanical Permit Electronics Permit Fencing Permit

✓ Clearances from other agencies Zoning and locational clearance Other necessary clearances from: DPWH ATO HLURB LGU DOT DENR DOTC DILG PPA DepEd DOH PHIVOLCS LLDA MWSS NWRB DAR DA DOLE NHA NCWDP

✓ Photocopy of PRC ID and current PTR Of all professionals who prepared and signed the documents

✓ 5. Five (5) certified true copies of OCT/TCT

✓ 6. Five (5) copies of Tax Declaration and current Real Property Tax Receipt
